Based on the information provided, I would place this student in a teacher-led small group (10-15 minute increments) where initially I would read aloud a short grade-level text as the students read and I would model thinking aloud about the text. I would stop frequently and ask myself questions to model active thinking and problem solving. I would read with emotion and be humorous where appropriate. I think this student is not truly engaging with the text if he is struggling to comprehend and respond to basic questions. I would spend several days using the same text in a small group setting to build familiarity and create repetition. I would gradually release the small group to student-modeled think aloud (student as “teacher”), and paired-reading while working to complete story maps with teacher-created questions. I might also include acting out of portions of the text and/or cartooning specific moments or scenes to further increase understanding and engagement. 

To extend vocabulary learning, I would also work to include words or memorable phrases from the text into the rest of the day/week so that he could hear that vocabulary being used in other contexts. I would encourage students to use those words with me. It creates a non-competitive challenge to see who can find or create opportunities to apply the vocabulary.
