Being able to identify the learning gaps of your students is important, because if they are not found early, the gaps can continue to expand and create more gaps in different areas. There are a few ways to identify gaps, whether it be through reviewing of testing data or passing out exit tickets at the end of a lesson you just taught. Then, after you have identified the gaps, you can start to make an action plan towards closing those gaps for the students, and how you want to plan future assessments. Identifying must be done with caution though, because you want to be sure there is a gap there before you start developing a plan. Be sure to use multiple methods to assist in identifying and understand that one bad test or review does not necessarily mean there is a gap.
